3. If you have ever held an educator license in Wisconsin, we will need to locate your licensing records. In your message, include the following identifying information:
   • Current full name including middle initial (and former name if applicable).
   • Educator file number (Displayed on your License Lookup record and printed on the upper left of your paper license certificate).
   • Date of birth. This will assist us in verifying that we have located the correct person in our data base.
