Consolidated End-of-Year Reporting

As a condition of receiving funding under the No Child left Behind Act of 2001 (NCLB) from the U.S. Department of Education, the state must report annually information under Title I, Part A; Title II, Part A; Title II, Part D; Title III, Part A; Title IV, Part A; and Title V, Part A. To comply with this requirement, we annually ask you to submit information related to the above programs through an End-of-Year Report.

The online Consolidated End-of-Year Report form for the 2006-07 school year is now available for completion. You may access the report from within the online ESEA Consolidated Application, or use the following link: https://www2.dpi.state.wi.us/ESEA_EOY/dpi_login/dpi_login.asp

Your userIDs and passwords are the same as for the ESEA Consolidated application.

The deadline for completion of the 2006-07 EOY report is August 30, 2007.
