Application Entry/Update Page - Scroll down to On-line Service to enter the FNS contract system and download a copy of the SFSP Internet Application Manual. If your agency is NEW to the SFSP, please visit the Application & Qualification Process for the Summer Food Service Program page. You will also need to contact Amy Kolano (608) 266-7124 prior to completing an on-line application.
Agreement/Policy Statement - PI-1482-AP Summer Food Service Program - Fillable. In addition to completing the on-line application all new sponsors must complete the Agreement/Policy Statement and return to DPI. Returning sponsors that have a change in the Authorized Representative of the Program are also required to complete this form.

The Wisconsin Free/Reduced Data for All Public Schools - September 2008.
This data report provides September 2008 enrollment figures, the number of children approved for free/reduced price meals and the % of children approved for free/reduced price meals in Wisconsin public schools. This information is used in the determination of Tier I eligibility for Family Day Care Homes and Summer Food Service Program sites.
