State Contract on Cell and Handheld Devices

Important: The following information applies only to schools and libraries which will be using the state Department of Administration (DOA) contract to receive discounts on (1) cell (voice) phone services, and/or (2) Internet services received via handheld devices. (Applicants always have the option of selecting the provider of their choice, outside of the state contract.) If you will be purchasing services from the DOA contract note the following:

  1. To receive discounts on only voice cell phone services applicants do not have to file a Form 470 but they do have to file a Form 471.
  2. To receive discounts on Internet services (e.g., email, Web) received via any type of handheld device (cell phone, BlackBerry, etc.) applicants must file both a Form 470 and Form 471. (See Handheld Devices below for more information.)

arrow 1) Background TOP

As of December 1, 2007, the state Department of Administration (DOA) has a new contract for services provided by cell phones and handheld devices. See the DOA cell contract Website for details. This contract can be used to apply for E-rate discounts for 2008. Under SLD E-rate rules the DOA contract is regarded as a State Master Contract (SMC). Under the SMC process, DOA submitted a Form 470 for voice cell services (but not for Internet services received via handheld devices) prior to starting the bid process earlier in 2007. The following vendors will provide services under the new contract. See the map of state regions for specific coverage and use the links below to review information each provider has on its various service plans, costs, etc. Schools and libraries are "authorized users." Some services and costs that apply to state government agencies may differ from the services and costs listed for authorized users.

arrow 2) Cell (voice) Services Only: 471 Block 5 TOP

Complete the information requested on the 471, blocks 1-4, in the usual manner. Below is the information to enter in the 471 Block 5. Some information below is pre-populated if you apply online.

Add New Funding Request - Search for Service Provider

Be certain Telecommunications Service is selected. Then enter the SPIN for your provider, Search for provider and Accept the results.

10. Duplicate Funding Request This applies to very few applicants and thus is almost always left blank.
11. Category of Service Be certain Telecommunications Service is checked.
12. Form 470 Application Number Enter DOA 470 number: 153990000609552.
13. SPIN - Service Provider Identification Number Accept SPIN [pre-populated].
14. Service Provider Name Accept provider's name [pre-populated].
Item 15a. Funding Request is for non-contracted service Leave blank.
Item 15b. Contract Number

Enter contract number. Use the U.S. Cellular contract number also for companies dba U.S. Cellular.

  • Cellular One contract: 15-91575-702
  • U.S. Cellular contract: 15-91575-701
  • Verizon Wireless contract: 15-91575-703
Item 15c. Funding Request is covered under a master contract Check box.
Item 15d. Check this box if this Funding Request is a continuation of an FRN Leave blank.
Item 16. Billing Account Number (e.g., billed telephone number) Enter appropriate number.
Item 17. Allowable Vendor Selection/Contract Date

This is based on the DOA Form 470 filing. Accept date of 01/16/2007 [pre-populated].

Item 18. Contract Award Date

For Cellular One and U.S. Cellular, enter: 11/30/2007.
For Verizon, enter: 12/20/2007.
(Note: This assumes you used the DOA 470. If you file your own 470, be certain the contract award date is after your Allowable Contract Date (i.e., after 28 days).

Item 19. Service Start Date Enter date. For most applicants this will be 07/01/2008.
Item 20a. Service End Date Leave blank.
Item 20b. Contract Expiration Date Enter: 06/30/2010.
Item 21. Description of This Service Give a short description of services. There is no need to submit the contract, just list the contract Website at : www.doa.state.wi.us/docview.asp?docid=6642.


arrow 3) Handheld Devices and Internet Access TOP

If you want to use the DOA contract to receive discounts on Internet services (e.g., email, Web) received via any type of handheld device (cell phone, BlackBerry, etc.), you must file both a Form 470 and Form 471. (The DOA 470 was filed only for voice cell services, not Internet access.) In completing the 470, note the following:

  • In Block 2, #7, check a, b and both boxes under b.
  • In Block 2, be certain to check #9, Internet Access.
  • If you want to receive discounts on both traditional (voice) cell services and Internet Service under the DOA contract, you may want to file your own Form 470 for both services. In this situation, to request funding for voice cell services on your 470, be certain to also check #8, Telecommunication Services, in addition to #9, Internet Access.
  • After waiting the required 28 days, if no vendor has responded to your 470 with a better offer than provided via the DOA contract, then file your 471. For Item #12 on the 471, enter the number of the Form 470 that you filed.

arrow 4) Companies Doing Business As (dba) U.S. Cellular TOP

The primary U.S. Cellular SPIN is 143011191. However, there are many other companies doing business as U.S. Cellular. The following is a list of these companies that DPI is aware of which will honor the DOA contract with U.S. Cellular. If you are getting services from one of these other companies, then list it on your 471 but use the U.S. Cellular contract # of 15-91575-701.
