Instructions for Using the Online LSTA Budget Revision Request Form



a. Any change to be made in the course of project implementation requiring transfer of funds between lines or a change in program focus must be requested in writing and approved by DLTCL prior to making the change. A Budget Revision Request Form must be used. All changes and budget revisions for 2009 projects must be made before December 11, 2009.

b.Go to the DLTCL form web site. You may want to bookmark this site. You will need Acrobat Reader to use the form. (If your computer does not have Acrobat Reader installed, you can download the latest version from Adobe.)

c. Enter your User Name and Password. Public libraries and systems were provided user names and passwords for filing the public library and system annual reports. If you have any questions about your password or user name, please contact Terrie Howe (608/266-2413); teresa.howe@dpi.wi.gov).

d. Once you are at the Forms Catalogs, click on LSTA Program.

e. From the list of LSTA forms that appears, select PI-2443 Budget Revision Request.

f. The first time you access this form there will be no preexisting files to open. Select New File, No Profile.

g. IMPORTANT: Click on the Save button and provide a file name for this application. You must use the library or system name and project number (e.g., Arrowhead 09-118) as the file name. The file name you use will appear on the center top of the first page of the form. The form can be printed anytime after you have named the file.

h. Complete the form, saving periodically. If you wish to close the form and complete it later, save your work and exit the form. The next time you access the LSTA Application folder, you work will be contained in the file named by you.

i. When you are ready to submit the form, TYPE IN YOUR NAME AT THE BOTTOM (Name of Authorized Agency Personnel) and click on the Route button on the top. You will receive confirmation that the form is ready to be submitted to the Division. Click on Finished to submit your form.

j. Print a copy of the completed form for your records. You do not need to submit a paper copy of the form.
