Section PI 9.07 of the Wisconsin Administrative Code requires that each school board file annually a pupil nondiscrimination compliance report with the Department of Public Instruction. The reporting form PI-1197, is a form-fillable Internet application and is due December 28, 2007. The form can be accessed at http://www5.dpi.state.wi.us/survey/0026/0026_pupil_nondis.htm.
The form consists of two parts:
Part I requests general information, including the name, address and telephone number of the school district employee designated to receive complaints of pupil discrimination filed under s.118.13, Wis. Stats.
Part II requires information about pupil nondiscrimination policies and procedures and discrimination complaints received by the district the preceding year, including the types of complaints received and the status of each complaint. In this section, complaints of written and verbal discrimination and harassment should be noted, including those that were handled informally or were withdrawn.
