Remember, you can save and return at any point!

Step 6: Complete Section I, Enter Contact and Completion Information.

You will need to enter general information:

  • The name of the primary contact or person completing the report
  • The phone number and e-mail address of the person completing the report
  • Whether the LEA's ad hoc self-assessment committee included a parent
  • Whether the LEA completed the student records review per WDPI protocols, which include sample selection and the use of an ad hoc self-assessment committee. If not, an explanation.

Step 7: Complete Section II, Enter Self-Assessment Results.

You will need to transcribe or fill-in the aggregated data from your handwritten or printed copy of the report into the electronic reporting system.

The program will calculate when you'll need a corrective action plan by automatically identifying compliance requirements.

After you've entered all aggregated data and the raw data for items not requiring samples, then you're ready to take a break from the computer and meet with your ad hoc self-assessment committee.

Step 8: Save and exit the electronic reporting system.
