Wait until your next license application to the department and just provide your current name and address on the application form at that time. There is a place on the application form to include your previous name(s). I was recently married and have a new last name. Can I get my license certificate re-issued with my new name? Your license is still valid under your previous name. However, if you do want a new license certificate reflecting the new name, you must complete the PI-1602-DUP form to request an updated duplicate license. There is a $50 fee for a duplicate license. I changed my last name this year. Do I need to get a new license certificate? No. Your current license is still valid under your previous name. When it is time to renew your license, provide your new name and previous name on the license application. Your name (and address if applicable) will be changed on our records at that time and the license certificate issued then will reflect your current information. I was recently divorced and have a different name and address. Is it acceptable to leave my license under the old name until I am due to renew? Yes. Your license certificate is valid under the previous/married name. However, if you wish to have the DPI educator license database records updated with your current name and address, you may complete the PI-1602-DUP form. There is a $50 fee for this service.
