If you have ever held an educator license in Wisconsin, we will need to locate your licensing records. Be sure to include your current full name including middle initial (and former name if applicable). If available, provide your file number (printed on the upper left of your license certificate and displayed on your record on the License Look-up. Providing your date of birth may assist us in verifying that we have located the correct person in our data base. Provide a telephone number where you can be reached during the business day in case we need to contact you for further identifying information.
