Educator License Application FormsHow to Open and Complete a PDF Fill-in FormAdobe Acrobat Fill-in pdf forms do not allow you to "apply on-line" for an educator license. Fill-in forms do allow you to enter information into an application form while it is displayed on your computer screen and to print out a completed form for mailing to DPI. The forms provide a high quality printout for your records and are easier for the department to process. Open the Application Form When you click on a selected application form title/number, the PDF application will open in Acrobat Reader. Read and follow all application instructions included in the PDF form. Page 1 contains instructions for the applicant information required on page 2. Instructions for the mandatory Conduct and Competency Review Form (page 3) are found on page 4. View Form Fields and Enter Information You may enter information in the form while it is open in the browser or you may download and save a blank form to disk and complete it later. The form may be completed on your computer, by hand, or a combination of the two. You may print a blank form while viewing it in the browser, and then complete the form by hand using black or blue ink. On the Acrobat toolbar menu you will see a hand tool which is a cursor shaped like a hand. Select (click on) the hand tool and use it to move the form around on your screen. By "clicking and dragging" you will be able to view the entire page. (To move to the next page of a form, use the scroll bar on the right of your screen.) When you position the hand cursor in a form field and click, the I-beam cursor will appear. The I-beam cursor allows you to type text into a field. Press the Tab key to accept the field contents and move to the next field. To accept a field and move to a previous field, use Shift plus Tab. The application forms contain some questions which require you to check an appropriate box. When you move the hand cursor over these fields, it will become a hand with a pointing index finger. Clicking on a box when the pointing finger is showing will put a check mark in that box. A check mark made in error can be removed by clicking on the same box again. Print a Copy of the Application before Closing Acrobat Reader Please carefully review your application as you are responsible for the accuracy and completeness of all required information. There is no automatic validation, verification, or editing of the information you enter. IMPORTANT: You will not be able to save a completed fill-in form with Reader software. The form will be saved but all entered information will be lost. You must print a paper copy before closing/saving the application form! When you are finished filling out the fields on the application, use your mouse to select an area of the form outside of a form field before attempting to print the form. This is important because if a form field is active (has a blinking bar in it), the contents of that field will not print. If the application form is displayed within your web browser's window, be sure to use the print button on the Acrobat toolbar menu (instead of the web browser print function) to print the form. Remember, entered information cannot be saved with the form. Do not close the application before printing or your entries will be lost!
