Educator License Application Forms

Install or Update Acrobat Reader

For best results, we recommend downloading the newest version of Acrobat Reader from Adobe's website. With older versions of Acrobat Reader, you may experience problems downloading the application forms and/or using the fill-in fields.

NOTE: Using Acrobat Reader, you will not be able to save a fill-in form containing entered information. You must print a paper copy of your application before closing the form!

If you are unable to download the pdf form(s), you may request that an application be mailed to you from Educator Licensing. Please specify the PI application form number in your request.
