Event Description
Braiding ACP (Academic and Career Planning) components with initiatives that affect students with disabilities will provide a catalyst for their successful transition. District teams will use the Transition Improvement Plan (TIP) to examine their district’s post school outcomes and researched based transition opportunities to increase post school outcomes for students with disabilities. They will create a coordinated and systematic plan that ensures students with disabilities have access to the activities and assistance needed to develop a career identity that link to the world of work and post secondary goals in the Post Secondary Transition Plan (PTP).

Develop ACP and transition resources to use for transition programming
