The Educator Effectiveness (EE) Grant and Peer Review Mentor Grant (PRMG) are state grants, and recipients must submit claims to be reimbursed for costs under the programs. EE and PRMG claims are made using the Program Fiscal Report (PI-1086) form. The Wisconsin Department of Public Instruction (DPI) provides instructions for the completion of the form .
Reported expenditures on the claim form must match the approved budget on file with the DPI program office, in this case the Educator Development and Support (EDS) team. Completed claim forms should be sent to the address listed on the form or to the DPI accountant assigned to the grant. The DPI School Management Services (SMS) team maintains a list of staff assignments.
Final claims forms must be received by DPI no later than September 30, 2018 to be processed. Failure to submit the form by the deadline will result in loss of funds.
