Wis. Admin Code s. PI 34.041 (4) requires educators who hold a valid life/lifetime license to complete an application to have a new background check conducted at least once every five years.
Educators with a valid life/lifetime license who have not completed a background check since January 1, 2015, need to complete a new background check before June 30, 2019. See the Background Check Requirements web page for details about requirements and how to determine your last background check date.

This page also provides information for educators who hold multiple lifetime licenses, a checklist to ensure you have completed all appropriate steps of the application, and directions for how to log into the system.
NOTE: At this point in time, PI-1613-Employment Verification forms are an OPTIONAL attachment for the Background Check to Maintain Lifetime License application.
