How to Apply for your GOALS Certificate of Excellence

 
 
Step 1: You'll first need to determine which track to follow by reviewing each track's eligibility requirements detailed on the GOALS Certificate Levels webpage. There is also a GOALS Track Decision Tree available to aid in the decision process. 
 
 

Step 2:  The next step is to review the specific training requirements related to the certificate level for which you are applying. You may choose to use the GOALS Mapping Worksheet to aid in creating a plan for meeting those requirements.

  • Acceptable trainings for the GOALS Certificate of Excellence must be sponsored by the Wisconsin Department of Public Instruction School Nutrition (DPI SNT), The Institute of Child Nutrition (ICN), or USDA’s Food and Nutrition Services (FNS).
  • All trainings you submit within your application must have been completed within the last 5 years. Other trainings may be credited with DPI approval. This is to ensure that the content of the trainings is accurate and based on current regulations.
 
Step 3:  You are required to map your training progress by tracking your completed trainings on the GOALS Training Tracker. Once you have obtained all of the correct number of training hours, you will submit the tracking tool along with copies of the documentation for creditable courses. Acceptable documentation includes training certificates, college transcripts, certifications, and other supporting documents with approval from DPI.

Step 4: Before you submit your application, please go through the following checklist and make sure the following is true regarding your application:

  • https://upload.wikimedia.org/wikipedia/commons/thumb/9/90/Check_mark_23x20_02.svg/1081px-Check_mark_23x20_02.svg.pngAll trainings I have completed were offered through DPI, ICN, USDA FNS, or were previously approved by a DPI staff member.

Step 5: Once you have completed the training requirements for your track, please gather the following: 
 
  • Completed GOALS Training Tracker
  • Certificates of Completion, email confirmations, or other documentation showing proof of completed trainings
  • If applying for a short-track, please include a resume with applicable experience (work, schooling, etc.)
    • If you are required to complete GOALS as a corrective action plan, please include documentation of communication with Karrie Isaacson.
Then submit all documentation above to GOALS@dpi.wi.gov or mail to:
