Complaints and Feedback

Complaint and feedback procedures:
- Do not dispose. Hold the affected cases in inventory away from other useable inventory. If the product must be disposed of for safety reasons, be sure to capture all issues in writing and take multiple photos of issue and packaging before disposing.
- If a foreign object is found, be sure to keep the object – do not dispose. The manufacturer will most likely ask for the object.
- Take multiple photos that show the issue and the extent of the issue.
- Take multiple photos of any packaging labels and dates.
- Complete as much of the DPI PI -6005 USDA Foods Program Complaint Form as you can. Our team and USDA will use the information to research the issue.
- Email your complaint or feedback to dpisnusdafoods@dpi.wi.gov. Attach the completed Complaint Form, photos, and other documentation.
- The Wisconsin DPI USDA Foods Team will keep you updated, and we will let you know when to dispose of the product.
