The Postsecondary Transition Plan (PTP) application was designed to assist individualized education program (IEP) teams in meeting the postsecondary transition planning requirements specified in 34 CFR 300.320(b) of the IDEA Regulations and section 115.787(2)(g) of the Wisconsin Statutes. The PTP is a required component of any student’s IEP in effect when the student is 14 and must be updated at least annually thereafter. As a legally mandated component of a student’s IEP, the PTP is to be developed, reviewed, and revised as appropriate, as part of the regular IEP team meeting process. A copy of the PTP must be provided to the parent along with all other required IEP documents.
