Important Notice

 

There have been many recent changes to licensure in statute and rule.  Please review the Latest Licensing News blog for updates on current licensing requirements.

 

Any person employed by a school district as a School Occupational Therapist (OT), School Occupational Therapy Assistant (OTA), School Physical Therapist (PT) or School Physical Therapy Assistant (PTA) must hold the license issued by the Wisconsin Department of Public Instruction.

License Application Requirements

 
To apply for a School Occupational Therapist, School Occupational Therapy Assistant or School Physical Therapist license, an applicant must hold a valid license from the State of Wisconsin Department of Safety & Professional Services (DSPS) in the area requested.  Please upload a copy of the DSPS license or wallet card with the application.
 

To apply for a School Physical Therapy Assistant license, the applicant must have graduated from a Physical Therapist Assistant associate degree program accredited by the Commission on Accreditation in Physical Therapy Education.  Please upload a copy of the transcripts confirming an associate degree in the area of Physical Therapy Assistant.

License Renewal Requirements

 
To renew a School Occupational Therapist, School Occupational Therapy Assistant or School Physical Therapist license, an applicant must continue to hold a valid license from the State of Wisconsin Department of Safety & Professional Services (DSPS) in the area requested. Please upload a copy of the DSPS license or wallet card with the application.
 
To renew a School Physical Therapy Assistant license, the applicant must continue to remain licensed as a Physical Therapy Assistant through the State of Wisconsin Department of Safety & Professional Services (DSPS) in the area requested. Please upload a copy of the DSPS license or wallet card with the application.
