Report Card Selection: WISEdata

Choice schools should have a current selection identified (i.e., opt-in or opt-out ) regarding the Private School - All Students Report Card. This selection is submitted via a form. The form can be located on the Private School Choice Programs: School Submitted Reports and Forms webpage, in the right-hand column, “Other Reports and Forms.”

Form submissions are due by the first weekday in August. Schools new to the Parental School Choice Program must select to either Opt In or Opt Out. Continuing schools only need to submit a new form if they want to change their previous selection. Selections cannot be changed once submitted for the current school year. Schools can change their selection for the following year.

Code Description/Comments

No Choice school has not opted in for the All Students Report Card.

· Opt-in (Yes) to the All Students Report Card requires ALL students (Choice and non-Choice participants) to be submitted to WISEdata and reported on the School Report Card.

· Opt-out (No) from the All Students Report Card requires Choice participants only to be reported on the School Report Card*

*Opting-out of the All Students Report Card does not allow for the use of the Wisconsin Immunization Registry Integration tool, while opting-in to the All Students Report Card allows schools to automatically pull down immunization data from the Wisconsin Immunization Registry if the SIS vendor participates.
