The Department of Health Services (DHS) is seeking your input to develop a statewide program to support school-based COVID-19 testing for teachers, staff, and students. The attached survey will assess your interest in various types of testing programs to inform the development of a statewide program best suited to the needs of schools in Wisconsin. Answers to this brief survey are nonbinding and will not be treated as an official commitment to participate in this program.
