Educator Effectiveness Annual Survey

Dear District Administrators,

The educator effectiveness (EE) grant requires participation in an annual survey. The Department of Public Instruction’s (DPI) Licensing, Educator Advancement and Development (LEAD) Team has developed a brief survey to replace the prior annual evaluation conducted by the University of Wisconsin – Milwaukee. This survey is required to be completed by one person at all school districts and charter schools who receive the educator effectiveness grant, regardless of which model they may be using, by June 16, 2023.

The DPI’s survey is shorter than prior surveys, with 21 multiple choice or check box questions focused on the implementation of the 6 EE Requirements. The DPI survey is designed to provide regional information that will be shared with DPI-contracted educator development and support staff at each of the 12 cooperative educational service agencies to improve supports around required elements of the system. The survey does not require school districts or charter schools to identify themselves, distribute questions to teachers, or provide demographic information.

How to take the Survey:

  • Assign one person to take the survey for the school district or independent charter school.
  • Open the Qualtrics survey.
  • Answer questions from the perspective of the whole school district or independent charter school based on general practice and policy.  Please note that some of the language of the questions may not match the specifics of each model, for instance the names of EE conferences (planning session versus “beginning of the year”) or the names of observation events (“mini-observations” versus “rapid-cycle”). In these cases, respondents should give the response closest to their policies or model.
  • Submit the survey. No other action is required.

Optional Resource

School districts and independent charter schools have requested survey questions that may be used locally. DPI is providing you with a list of questions used in prior years as a resource. Schools may use these questions for their local purposes, but these are not required by the DPI.
