Recognition of Commitment and Service

About the process
Upon request, the department produces certificates to honor those who have served in the education field. This covers any staff working in the school setting, including teachers, administrators, paraprofessionals, food service, grounds staff, support staff, etc. All requests for certificates must come through this form. The certificates are customized for each retiree, are printed on parchment paper, contain the state superintendent's signature, and have a gold state seal. The department is continuously collecting requests, and produces certificates on the first and the 15th of every month.
Retiree information
Please use the following fields to provide the details needed to create a certificate from the state superintendent honoring their years of service to the education field.
If you are requesting 10 or more certificates, you can upload a file using the template provided at Recognition of Commitment and Service Template. If you do not use the template provided, the department cannot produce the certificates you're requesting.
Files must be less than 20 MB.
Allowed file types: xlsx.
Please provide the first name of the retiree you're looking to honor as you want it to appear on the certificate

Please indicate the years the retiree has served in the education field
Date preferred on certificate
Certificates will be dated the day they are printed unless a specific date is provided.
