The Transfer of Service (TOS) portal opened May 20th and there have been some very good questions this year that we compiled them into a questions and answers document. Even if you have been doing TOS requests for years, we suggest you review our new Transfer of Service Frequently Asked Questions document. The document helps clarify the parts that must be met to qualify for TOS.
Submissions of new TOS requests through Part A of the PI-5000 are due Friday, September 17. Detailed information is available in our Directions for Completing Transfer of Service document.
